Declar or Change Major

Declaring your changing your major to Community Health is a two step process:

First, go to the college Student Services website hhp.umd.edu/studentservices.

- Under "Current Student" click on "The 4-Year Plan"

- Read and at the bottom of the page click on the "here" which will take you to the "Directions for       Completing the Academic Plan" webpage

- Follow the directions -- print and complete the two "HLTH" sheets:

      "Academic Planning"

      "Community Health Four Year Template"

Second, after you have completed the above, go to http://www.flashappointments.com/client/launch.cfm?busId=4082 http://www.flashappointments.com/client/launch.cfm?busId=4082. to schedule an advising appointment to see Dr. Hyde. Bring the two forms with you - make sure you complete each one.
